TJC reports that this EP is scored in just under 6% of hospital surveys. But, our observations, from mock surveys in outpatient and procedural settings, as well as among pediatric populations, indicate that this may be missed more than 6% of the time.
TJC makes their guidance clear. This requirement applies in all patient care areas, not just inpatient. Yet we often find outpatient settings where the EMR fields do not correspond to the inpatient area. In fact, the routine screening question or observations are not prompted.
Also, pediatric settings pose a challenge. Many organizations use a screening question such as, “Do you feel safe at home?” But, such a screening question is not adequate for a nonverbal child. Thus it requires some observation-based screening.
