Titration orders typically involve adjusting the dosage of medication either upward or downward based on the patient’s condition. It is mandatory for organizations to establish policies that determine whether such orders are permissible.
If a titration drip rate is zero, is a new order required to restart it?

If the nurse followed the therapeutic order and the clinical presentation justified a zero rate, the nurse could restart the drip per the titration order if the clinical condition again changed. There is a caveat, you need to ensure that the order duration in your automatic stop order policy was still valid. Also, If a physician had said, “Stop the titration,” a new order would be needed.
